Thought record planner template

I made a simple thought record template for my personal planner, and I thought I'd share! This is my take on a thought record and thought challenging tool, as used in cognitive behavioral therapy. I might make a prettier version later, but for now, this was quick and effective. The file is letter size and meant to be printed and cut in half to make two pages for use with a half-letter planner. The top right line is for filling in the date. PDF and ODT versions attached, or you can click the preview image for a PNG.
